Combined Use of Pleural Fluid Adenosine Deaminase (ADA), Cytology, Pleural Biopsy and Mantoux test In the Diagnosis of Tuberculous Pleural Effusion

چکیده
The objective of the present study is to evaluate the use of pleural fluid ADA, cytology, pleural biopsy and Mantoux test in the diagnosis of tuberculous pleural effusion. This prospective study was conducted on 92 patients December 2011– October 2013 at Government Chest Diseases and Tuberculosis Hospital, Hanamkonda. Diagnostic accuracy of pleural fluid cytology (L/N ratio >0.75) , ADA >40 U/L , Pleural biopsy and Mantoux test were 65.22%, 73.91%, 92.13%, 65.22% respectively .Combined pleural fluid ADA , Cytology , pleural biopsy and Mantoux test results were analyzed sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy were 93.33 %, 93.75%, 96.55%, 88.23% ,93.48 % respectively in the diagnosis of tuberculous effusions among all exudative effusions were better than individual alone. Lymphocyte predominant exudates with high Adenosine deaminase value > 40U/L & caseous granuloma on pleural biopsy are highly suggestive of Tubercular pleurisy. Combined use of ADA, Cytology, Pleural biopsy and Mantoux test are more helpful in diagnosing Tubercular pleural effusions rather than individual alone.

Background & Aim: Pleural effusion is seen in the background of many diseases, two major groups of which are tuberculosis and malignancies. At present pleural biopsy helps us differentiate these two causes from each other, but it is not only an invasive method but also an expensive one. For this reason, investigators are in search of simple and less invasive methods to diagnose the cause of...
متن کاملValue of adenosine deaminase in the diagnosis of tuberculous pleural effusions in young patients in a region of high prevalence of tuberculosis.
BACKGROUND Pleural biopsy is usually considered important for the diagnosis of pleural effusions, especially for distinguishing between tuberculosis and neoplasia, even though tuberculous pleural fluid contains sensitive biochemical markers. In regions with a high prevalence of tuberculosis, and in patient groups with a low risk of other causes of pleurisy, the positive predictive value of thes...
متن کاملCombined use of pleural adenosine deaminase with lymphocyte/neutrophil ratio. Increased specificity for the diagnosis of tuberculous pleuritis.
UNLABELLED Increased pleural fluid adenosine deaminase (ADA) activity is classically associated with tuberculous pleuritis. However, increased activity can also occur in a number of other diseases and this may negatively affect the diagnostic utility of ADA measurements and decrease its specificity for the diagnosis of tuberculosis (TB). The presence of ADA in pleural fluids reflects the cellul...
متن کاملDiagnostic value of adenosine deaminase activity in tuberculous effusions.
We read with interest the recent paper by QUEROL JM, BARBE F, MANRESA F, EsTEBAN L and CANETE [1]. The authors presented some patients with tuberculous pleural effusion and adenosine deaminase activity (ADA) values less than 43 U·I-• We reported previously that ADA values (Guisti method) were statistically significantly higher in tuberculous pleural effusions than in most other diseases [2]. Hi...
متن کاملDiagnostic Accuracy of Adenosine Deaminase and Lymphocyte Proportion in Pleural Fluid for Tuberculous Pleurisy in Different Prevalence Scenarios
BACKGROUND Tuberculous pleural effusion (TPE) is a paucibacillary manifestation of tuberculosis, so isolation of Mycobacterium tuberculosis is difficult, biomarkers being an alternative for diagnosis. Adenosine deaminase (ADA) is the most cost-effective pleural fluid marker and is routinely used in high prevalence settings, whereas its value is questioned in areas with low prevalence. The lymph...
